LIABILITY WAIVER

Liability Waiver and Acknowledgment of Risk Form 

MUST BE READ AND SIGNED BY A PARENT OR LEGAL GUARDIAN OF EACH CHILD PRESENT   

Being around ponies and horses and riding ponies and horses are potentially dangerous activities. I fully understand the potential dangers and risks involved with myself and/or my child/children being around and riding ponies and horses, including but not limited to death or serious injuries associated with the head, neck, and back, which may result in complete or partial impairment of my or my child's body, general health, and well being. I hereby acknowledge these risks and all other risks involved in rural outdoor activities, which could result in injury, including paralysis or death, and I give my child/children (name/s)_____________________________________________________________________________

permission to participate, and I voluntarily assume all risks to myself and my child's/children's health associated with participation in these activities, and agree to discharge, release, and indemnify Vince, Jacki, and Hannah Cook, their relatives, GiddyUp Pony LLC (assumed business name Praise the Lord Pony Rides), their agents, servants, volunteers, and/or employees, from any and all liability in the event of an injurious accident or incident that occurs to myself or my child/children while on their property, as well as, any and all claims, causes of action, or demands of any kind and nature whatsoever which may arise by or in connection with my or my child's participating in pony rides or any other animal or non-animal activities offered by any of the foregoing on the premises of 11703 W. Gila Dr., Kuna, Idaho. The terms hereof shall serve as a release and assumption of risk for my heirs, estate, executor, administrator, assignees, and all members of my family.  I sign here to attest my full understanding of, consent to, and agreement with the terms of this liability waiver and acknowledgement of risk form.
